Fees and Insurance
Family Roots Therapy is a counseling practice in Portland, Oregon, focused specifically on the unique mental health needs of new parents and young children. We understand that early childhood and the adjustment to parenthood are critically important times in everyone’s life. We know that the mental health of parents and their young children are uniquely connected and must be treated with great knowledge and care.
We are in network with Aetna, BCBS, Cigna, MODA, OHP Open Card, CareOregon (OHP), Pacific Source (commercial), Providence, and Trillium OHP. We can now accept Tricare/Triwest. We need to be made aware of any and all active medical coverage even if we are not in network. Please contact us if you have any questions.
*Out-of-network insurance is not accepted. Payment of the full charge rate is required at the time of service.
*Due to contract limitations, we are unable to accept new clients with out-of-network primary insurance who also have OHP as secondary insurance.
Superbills can be issued for licensed providers ONLY and may be submitted by the client to their insurance company.We can no longer provide estimates of coverage and out-of-pocket costs. Please contact your insurance company to confirm your coverage for a specific provider or specific therapy services. Some insurance companies may not cover family therapy.
Time of Service Cash Rates
Sliding scale spots are limited and cannot be submitted to insurance for superbill reimbursement.
Mental Health Assessment (CPT 90791) $200
53 minute individual session (CPT code 90837) $175
45 minute individual session (CPT code 90834) $135
30 minute individual session (CPT code 90832) $100
Family Therapy w/ client present (CPT code 90847 – billed for 26+ minutes) $175 (CPT code 90847 – billed for 26+ minutes)
Family Therapy without client present (CPT code 90846 – billed for 26+ minutes) $175
Crisis (CPT 90839 - first 60 minutes of psychotherapy for crisis) $200
Crisis (CPT 90840 - each additional 30 minutes after first 60) $100
Case Management: billed in 15 minute increments at $35 per 15 minutes for contact with external resources or providers *not covered by most insurances
Neurofeedback: $105 per 45 minute session
Parent Coaching: $175/session *This cannot be billed to insurance
Good Faith Estimates are available upon request or after scheduling a first appointment for any clients who are not using insurance benefits.
